No Fault in the Switches: AI171 Investigation Narrows as Final Report Nears
Fourteen months after Air India flight AI171 crashed seconds after take-off from Ahmedabad, the investigation into India's worst aviation disaster in decades has reached a pivotal and unsettling point. In late July 2026, the Indian government told Parliament that a detailed examination of the Boeing 787's fuel control switches — the components at the centre of the inquiry since the preliminary report — found no defect, including in the locking mechanism designed to prevent inadvertent movement. And India's Aircraft Accident Investigation Bureau (AAIB) has told the Supreme Court that its final report is due in October 2026.
Taken together, the two developments narrow the investigation dramatically. They also make its conclusion harder, not easier, to write.
What happened on 12 June 2025
The established facts are stark. On 12 June 2025, AI171, a Boeing 787-8 Dreamliner bound for London Gatwick, lifted off from Ahmedabad's Sardar Vallabhbhai Patel International Airport and crashed roughly half a minute later into a densely built area near the airport, striking a medical college hostel. Of the 242 people on board, 241 were killed; one passenger survived. A further 19 people died on the ground. It was the first fatal crash — and first hull loss — of a Boeing 787 in the type's history.
The AAIB's preliminary report, released in July 2025, contained the detail that has defined the investigation ever since: seconds after lift-off, both engine fuel control switches transitioned from RUN to CUTOFF, one after the other, starving both engines of fuel at the most vulnerable phase of flight. The report also described cockpit voice recorder audio in which one pilot asked the other why he had cut off the fuel, and the other denied doing so. The switches were subsequently returned to RUN and one engine began to relight, but the aircraft had neither the altitude nor the time to recover.
From that moment, the inquiry has faced a binary that investigators everywhere dread: either the switches moved without a deliberate human action — implying a mechanical, electrical or software pathway — or a human hand moved them.
The July 2026 findings
The government's statement to Parliament on 30 July 2026 addressed the first branch of that binary. According to the Ministry of Civil Aviation, detailed examination of the fuel control switch assembly, including the structural integrity of the locking detents that require a switch to be lifted before it can be moved, found no abnormality. This matters because the mechanical-slip hypothesis had a documented hook: a 2018 FAA Special Airworthiness Information Bulletin had noted reports of fuel control switches installed on some Boeing 737s with the locking feature disengaged, and recommended inspection — advisory, not mandatory — across several Boeing types including the 787.
If the locking mechanism on the accident aircraft's switches was intact and functional, the "switch simply slipped" explanation becomes very difficult to sustain. Aviation analysts following the case note that this pushes the inquiry deeper into the electronics — the thrust control module and the signal path between the switches and the engines' full-authority digital engine controls, with hardware reported to be under bench examination with Boeing's participation under ICAO Annex 13 provisions, which give the state of design and manufacture a formal role in the investigation.
It is essential to be precise about what has and has not been established. Finding no defect in the switch hardware is not a finding of pilot action. The AAIB has released no conclusion on how or why the switches moved, and its officials have publicly cautioned against premature speculation — criticism that grew pointed in 2025 after some Western media reports suggested crew involvement before the evidence supported any such determination. The two pilots — both experienced, with the captain holding thousands of hours on type — deserve the presumption that attaches to every crew until an investigation concludes otherwise.
The Supreme Court and the timeline
The Indian judiciary's involvement is itself notable. Families of victims and public-interest petitioners approached the Supreme Court seeking assurances about the investigation's independence and pace; in mid-July 2026, the AAIB told the court that its final report would be ready by October 2026 — roughly sixteen months after the accident.
By international standards that would be a respectable timeline. ICAO Annex 13 sets an aspiration of twelve months, but complex modern investigations routinely take two years or more. The pressure on the AAIB is nonetheless immense: this is the highest-profile investigation the bureau has conducted since it was carved out as an independent body, and its handling of AI171 is widely seen as a test of India's maturing safety-investigation ecosystem — separate from the DGCA, the regulator, whose own oversight of Air India has drawn scrutiny since the accident.
What rides on the final report
The consequences of October's report extend far beyond one airline.
For Boeing, the stakes are the integrity of the 787's fuel-cutoff architecture. A finding implicating any electronic pathway between the switches and the engines would trigger airworthiness action across a global fleet of more than a thousand Dreamliners, and would arrive with Boeing still rebuilding regulatory trust after the 737 MAX crises. Conversely, a finding of deliberate or inadvertent human action would shift attention to cockpit procedures, crew monitoring and — in the most difficult scenario — the aviation industry's still-halting approach to pilot mental health.
For Air India, under Tata ownership and mid-way through an ambitious fleet and service transformation, the report will shape both liability proceedings and the carrier's safety narrative. For India's regulators, it will inform whether the DGCA's post-accident inspections and directives — including its checks of fuel control switch locking mechanisms across Indian operators' Boeing fleets in July 2025 — were proportionate.
And for the families of 260 victims, the report is something simpler: an answer. The preliminary report told them what happened in those 32 seconds. The final report must attempt to say why — and if the evidence cannot fully resolve the binary at the heart of the case, the AAIB will face the hardest task in accident investigation: saying so honestly.
The wider safety ledger
It is worth holding two truths at once. AI171 was a catastrophe, and Indian aviation in 2026 remains statistically far safer than it was a generation ago — a market carrying record traffic on a young fleet, under a regulator that has progressively aligned with ICAO standards. Accidents of this magnitude distort perception precisely because they have become so rare. The measure of a safety system is not that nothing ever goes wrong; it is what the system does next. In October, India's investigators will show the world their answer.
